2025 Deeper Dive - Fall Series

Webinar #1:  October 3, 2025

Critical Ethical & Relationship Considerations in Perinatal Palliative Care - Dr. Brian Carter

Brief Description:
Perinatal palliative care (PnPC) consultation requires attention to the unique phenomena of pregnancy among human experiences. While helping to address clinical matters, the PnPC consultant must establish trust and assure the family that they will be present along the continuum of care needs and locations for the mother, baby, and multiple caregivers across diverse care environments – upholding the palliative care goals of accompaniment and bearing witness.

Learning Objectives:

  • Recognize the uniqueness of every pregnancy and its narrative.
  • List 3 concerning reactions of parents to fetal diagnostic findings that require your attention in prenatal counseling and subsequent care.
  • Define helpful approaches to communication with families in prenatal consultations.

Webinar #2: October 10, 2025

Periviable Birth: Integrating Neonatology, Ethics, and Palliative Care at the Limits of Viability - Dr. Christopher Collura

Brief Description:
This webinar will review the current state of periviable birth including survival and neurodevelopmental outcomes, practice variability, and the anticipated neonatal intensive care for these infants. It will highlight historical and current ethical controversies in navigating the grey zone of viability including the potential for new treatments including the artificial womb. Opportunities for pediatric palliative care in the support of pregnant women, families, and the care team navigating periviable birth will be considered. It will highlight communication strategies to support shared decision-making and compassionate care for families facing uncertainty.

Learning Objectives:

  • Define periviable birth, neonatal viability, and extremely low gestational age in the context of current evidence, practice variability, and evolving outcomes.
  • Recognize the ethical challenges that arise when caring for pregnant women at periviable gestation and neonates at the threshold of viability.
  • Consider models for integrating palliative care in the NICU for the extremely low gestational age neonatal population.

Webinar #3: October 17, 2025

Prognostic Uncertainty in the Neonatal Period - Dr. Robert Macauley

Brief Description:

Prognostication in the neonatal period can be especially challenging. Gestational age may be uncertain, and even when it is not, there exists a wide range of possible outcomes. Some findings long thought to be predictive (such as intraventricular hemorrhage) are now viewed with greater skepticism. And tools frequently applied in adult medicine (such as the Palliative Performance Scale) have minimal applicability when all babies—not just those with serious illness—require significant support. Yet claiming “not to have a crystal ball” provides parents scant support in decision-making, especially when a “window of opportunity” may be closing to make pivotal decisions. This webinar will equip learners with tools and perspective to prognosticate appropriately and helpfully for their neonatal patients.

Learning Objectives:

  • Identify sources of prognostic uncertainty in the neonatal period
  • Delineate between reliable and unreliable predictive factors
  • Formulate a thoughtful approach to supporting families and teams in navigating prognostic uncertainty
